*Regular cost is based on the usual and customary rate charged by Smile Brands dental offices in the state of Washington.
**Plan cost represents the discounted price that OneSmile® Dental Plan members will pay when services are performed by a participating general dentist in Washington. Certain procedures may not be within the scope of practice or the ability of the general dentist. In such cases, if a participating specialist is available to perform such procedures, they will be provided at a discounted rate off the participating specialist's usual and customary fee for such procedures.
